2、《企业法人营业执照》副本复印件。 3、《中华人民共和国组织机构代码证书》复印件。 4、《国税登记证》复印件,《地税登记证》复印件。 5、注册资本验资报告复印件或有关注册资本来源的证明材料。 6上年末以及本年度上半年两个时点的企业财务报表。 7、法定代表人、总经理、财务负责人、本报表填表人身份证明复印件。 8、法定代表人、负责人履历证明材料;;企业受表彰情况、企业专利证书、国际认证、特殊行业资质的相关证明复印件等。 二、填表须知 1、本表中期初数指本企业上年度财务报表的年末数,期末数指本企业上一个月度财务报表的月末数。 2、联系电话、传真号码要求写上区号。 3、各表中所有涉及日期要求按年、月、日格式填写。 4、在“企业特征”项中应根据国经贸中小企〔2003〕143号文的要求填写“中型”/“小型”(根据该文整理的《中小企业标准一览表》见附件一)。 5、在“行业分类”中应根据《国民经济行业分类》国家标准(GB/T 4754-2002)填写“字母+3位数字”的中类代码(《国民经济行业分类-2002》)。 6、所涉及日期的要求按、年、月、日格式填写。 7、企业应在填写本表时仔细阅读各表下的注释。 8、在“最高学历”中应根据文化程度代码(GB/T4658-1984)填写代码(根据个人信用信息基础数据库系统数据接口规范整理的《文化程度代码》见附件二)。 9、在“持有企业股份”中应填写持有股份占比比例。 10、期初损益类项目填写上年度全年数。 11、资产类项目中的“10、其他长期资产”指除固定资产、长期投资以外的其他各类长期资产之和。 企业概况信息表(表一)填写说明 注:1. 企业名称:营业执照上登记的企业名称。2. 企业注册地址:营业执照上登记的企业地址。3.企业通讯地址:企业实际邮政通讯地址。4.邮政编码:通讯地邮政编码。5. 组织机构代码:指国家质量技术监督部门颁发的《中华人民共和国组织机构代码证》中的代码。6.企业注册登记日期:企业营业执照中的登记日期。7.企业特征:填写企业的规模,中型/小型。按照2003年国家经济贸易委员会、国家发展计划委员会、财政部、国家统计局联合颁布的《关于印发中小企业标准暂行规定的通知》(国经贸中小企[2003]143号)执行。8. 工商登记注册号:该企业在工商管理部门注册登记时的编码,即工商营业执照上的编码。9.登记注册类型:营业执照上登记的注册类型。10. 地税登记证号码:由地税局发给企业的税务登记证上的证件编号。11. 国税登记证号码:由国税局发给企业的税务登记证上的证件编号。12. 行业分类:依据《国民经济行业分类》国家标准(GB/T
